Move-In Instructions for Long Term Assisted Living Residents

Please complete and submit the following items:

• Application for Residency
• A medical report with a chest X-Ray (current within 6 months), a TB test (current within 90 days) and a list of current medications, to be completed and signed by your physician. The Hospitality Office can provide the appropriate forms for your doctor.
• Photocopies of your medical insurance cards
• Photocopies of your Living Will, Durable Power of Attorney, Power of Attorney and/or Guardianship papers

After selecting your apartment, submit your reservation form, your reservation deposit and your move-in date as agreed upon with community staff (we’ll work with you to schedule a convenient date and time).

Our Nursing staff will provide you with an assessment so you can begin receiving the appropriate level of care.
