Plain Language Summary - Financial Assistance and Charity Care Policy
In furtherance of its mission to provide assistance to those who are unable to pay for health care expenses, Trinity Health Senior Communities (“Ministry”) offers financial assistance to certain individuals who qualify for financial support at Ministry. Management of facility funds and eligibility for financial assistance is a complex process, so as a resident benefit, Ministry offers its residents this plain language summary of its Financial Assistance Policy/Procedure to summarize the important components. STEP 1: SERVICES ELIGIBLE FOR FINANCIAL SUPPORT
Ministry does not provide financial support for all services you may receive at Ministry. Instead, Financial Support is only available to qualifying residents who receive medically necessary services that are not paid for by another third party payor.
STEP 2: EXPLORING OTHER THIRD PARTY PAYORS
Prior to receiving financial support from Ministry, you must demonstrate that you have sufficiently attempted to obtain coverage from other available third party payors, such as Medicaid, and that you have exhausted all applicable third party benefit appeal processes. To the extent that you require services during the pendency of the application or appeal process for any third-party benefits, Ministry may extend financial support to you continent upon the approval or disapproval of your application.
STEP 3: APPLYING FOR FINANCIAL SUPPORT FROM MINISTRY
If the cost of your care is not covered by a third-party payor, you may wish to apply for financial support directly from Ministry. To do so, you will be asked to complete a financial support application, and you will need to disclose and provide evidence of your income and assets. You can find the financial support application here. Ministry offers financial support primarily based upon whether your income falls below a certain Federal Poverty Level threshold. Financial support is also available on a case-by-case basis for medically indigent residents and individuals who fall into certain presumptive support categories.
STEP 4: FINANCIAL SUPPORT DETERMATIONS
Every effort should be made to apply for financial support from Ministry prior to or at the time of admission or services. Financial support applications will be accepted, however, until one year after the issuance of the first billing statement that describes the service at issue. Once qualification for financial support from Ministry has been determined, subsequent reviews for continued eligibility will be made on a yearly basis.
