Our colleagues in the Admissions Center can help you with the application process, provide you with information regarding Heartwood Lodge and give you a tour of our impressive community. Our Director of Admissions will also perform an on-site evaluation of a prospective resident, whether at home, in an assisted living facility, a hospital, or another skilled nursing facility. Working with the hospital discharge planner, the family, physicians, and home care providers, we refer each resident or client to the most appropriate level of care at Heartwood Lodge.
