Move-in Process for Nursing Care

New residents may need to obtain a medical report with a chest X-Ray (current within 90 days) and a list of current medications, to be completed and signed by your physician. The Admissions Office can provide blank forms for your doctor. If the senior is at the hospital, medical records may be sent via FAX. We may also require payment verification information (insurance, private payment or Medicaid).

After receiving these materials, our Admissions Office will schedule a convenient move-in date and time. Be sure that a family member is present on your move-in day to sign admission documents, provide consent for services and copies of insurance cards and legal documents.

Once you’ve settled in, our Wellness Nurse will pay you a visit. Our Director of Community Life will ask about your interests and invite you to begin enjoying the exciting lifestyle at Sanctuary at Fraser Villa!
